(Contract No. 2018-0375) | | Background: | Due to the nature of the population, the majority of services should be provided in the home and during non-traditional hours. However, services may be conducted at the FYSC or JPD when deemed necessary by CSD staff and assigned JPO. Services must also be client centered and adhere to program guidelines. CSD shall provide two bachelor-level Care Managers to provide skills training, group educational class assistance, linkages and follow up to community based services, service record receipt, and care management services.
