| Subject: | Approve and authorize the electronic submission of a Texas Health and Human Services Community Partner Program Interest Form allowing the Community Services Department to participate in the Community Partner Program. (Contract No. 2018-0382) |
| Background: | The Community Partner Program would allow the Community Services Department to serve as a navigation site for those individuals wanting to apply for health (Medicaid, Chip & Healthy Texas Women), food (SNAP), and cash assistance (TANF). Individuals in the community would be able to come to our offices and utilize a computer to apply and renew benefits, upload documents, update their account and view the status of their benefits. Existing Community Services staff would assist individuals if necessary with navigation and application for HHSC benefits. The Community Services Department would be designated as a Level 2 Assistance Site (navigation site). The interest form requests the signature authority to sign a contractual agreement for the county if accepted as a partner. The interest form is to be submitted electronically. |
